Oracle Database Error Code ORA-16640 Description :
CRS warns that multiple instances may still be running
Error Cause:
A broker operation was underway that required CRS to stop monitoring the instances of this database and to shut down all but one instance. Although instance monitoring has ceased, CRS cannot guarantee that only one instance remains running. The broker operation was canceled.
How to Solve :
Suspend CRS management of this database using SRVCTL STOP DATABASE -D -O NONE. Then reissue the broker request.
